What information does HealthPal use?
Age, Ethnicity, Gender, Weight, Height, Tobacco history.
Access to State Vaccine Database, Access to Health Insurance Claims Data or dates of last Mammogram, Low dose CT, Colonoscopy, Cologuard (R), colon screening stool cards; information about when your doctor says you need to have your next screening.
Primary Care Provider name, phone number, date last physical, Other Provider name & phone number.
How can I sign up my child?
You will be able to track them until the age of 18 if you verify legal guardianship. After that, we will send a special birthday notification that includes a link to transition the account to their care.
What if I don't know what my colonoscopy interval is?
The best person to ask about this is whoever performed the first colonoscopy, probably a specialist. If they are not available or accessible, your Primary Care Provider should be able to determine this for you.
